IAAAA Education Institute, the educational arm of Indiana Association of Area Agencies on Aging advocates for quality programs and services for older adults and persons with disabilities.  We recognize the need to get important medical information to those that need it the most.

 

IAAAA Education Institute would like to inform you that over 2000 “Vial of Life” medical information storage kits were distributed at the Indiana State Fair during Senior Day on August 17, 2006.  

 

The kit contains:

 

1.     A Medical Information Form

2.     Vial with bright sticker on front to store the form and any advance directives the individual may have.  Note:  The vial should be stored on the top shelf in the door of the refrigerator.

3.     Magnet for the outside of the refrigerator in the upper corner to signal the presence of medical information inside.

 

The kit enables emergency responders, such as firefighters, paramedics and police, in the local communities to quickly locate helpful medical history information.  It also includes important information about Medicare Rx and who to call to report suspected Medicare/Medicaid Fraud, Waste, or Abuse. Our hope is that the individuals will use the kit as instructed and it will assist with the prompt delivery of emergency services.

 

The Vial of Life is a collaboration of the IAAAA Education Institute’s Senior ESP Program, Access to Benefits Coalition and Hoosier Rx.
